is a social enterprise not-for-profit limited company.

Sequeli's aims are to promote and improve learning of lessons following serious untoward incidents in the children, adults, mental healthcare and criminal justice services in England, Scotland and Wales.

Sequeli pursues these aims by

  • providing training, a training curriculum and training materials for chairs, report writers, expert panel members and others involved with reviews and investigations
  • supporting those conducting and commissioning reviews and investigations through supply of information, comment, links and resources, including its training curriculum Core Competencies, on this Sequeli website and on Sequeli's interactive website SequeliNetwork
  • providing information on the process of learning lessons generally, for all visitors to this site
  • gathering source material on the ways 'learning lessons services' operate, for publication, practice, planning and policy use, through Sequeli's survey of review and investigation methodologies in England and Wales.

Sequeli and SequeliNetwork are the trading names of Sequeli Limited.

Sequeli people

Sequeli was formed in November 2010 by co-directors Professor Roger Bullock and Gillian Downham with Associate Wendy Rose. In August 2011 they were joined by Professor Christopher Heginbotham who became an Associate. We are very pleased that Professor Jill Manthorpe, James Blewett and Dr Androulla Johnstone also became Associates in 2013. Each of them has given their time and commitment to supporting Sequeli's goals.

Between them the Directors and Associates have legal, research, national strategy and academic backgrounds with cross-boundary experience of reviews and investigations. Read more about them on our Sequeli Profiles page.

We make special mention of King's College London and 1 Garden Court Family Law Chambers Temple London who have supported and encouraged Sequeli since its inception.

Sequeli thanks all who are part of its organisation, including its advisors and partcipants in its training. We look forward to continued partnership as Sequeli steadily grows.

Working with experts

Sequeli is delighted to have been working during 2013 and 2014 with Sequeli Associates Jill Manthorpe and James Blewett in partnership with colleagues at NSPCC and Action for Children on the Department for Education 'Improving the Quality of Serious Case Reviews' training programme. Sequeli produced the training materials for this contract and the work opened valuable channels of communication with the Department for Education whose very helpful advisors supported the programme.

We were fortunate to have had a similarly rewarding partnership with Laura Richards Consultancy during 2012 and 2013 working on a contract with the Home Office for provision of Training Materials for Chairs and Report Writers of Domestic Homicide Reviews, helped by insightful contributions from Frank Mullane of Action After Fatal Domestic Abuse (AAFDA). Again, working closely with the helpful Home Office advisors was key to the success of this project.

We are pleased that Daniel McCarthy joined Sequeli in 2013 as an Advisor on Training and Development. With his experience of advising on and developing models for national learning, he is helping Sequeli consolidate its place as the leading national provider of integrated training on reviews and investigations in the public sector.

We are particularly grateful for the encouragement we have received from the families of victims and perpetrators. We share their hope that ultimately this will improve reviews and investigations for all.
